Follow these easy steps to sign up for the Patient Portal

  1. Request a security token number and sign-up instructions from the reception staff at any of our health centers.
  2. Once you request a security token, we’ll give you a Reference Guide and send you an email with a link to the Patient Portal registration page where you can choose your username, password and security questions. Your token number will confirm your identity and allow us to complete your account setup, so keep it handy.
  3. That’s it! Now you’re ready to access the Patient Portal. Use your computer or cell phone to visit Follow the steps in your email, or in the Patient Portal Reference Guide, to complete your enrollment and gain access to your account.
