Our health centers grapple with the most fundamental health problems besetting our communities. Almost one-third of our patients have some form of chronic illness, and the overwhelming numbers of these individuals have multiple disorders.
Our patients, who range from babies to older adults, are offered comprehensive primary care medical services including physical exams, chronic disease management services, health maintenance support, vaccines, immunizations, incision/drainage of cysts, outpatient HIV testing and care, well-child care, CHDP exams, addiction medicine, screenings for anemia, lead, vision, hearing and tuberculosis. We also have a pharmacy onsite. Services are offered in both English and Spanish.
Other special clinical initiatives include offsite services to populations with special barriers to care including seniors in skilled nursing facilities and the homeless; an organization-wide pursuit of solutions to management of diabetes, HIV and early cancer detection; and the development of collaborative responses to wide-ranging health threats such as MRSA.
To address difficult problems like addiction, mental illness and chronic pain, along with lifestyle diseases like tobacco abuse and obesity, MCHC utilizes the combined efforts of Primary Care and Behavioral Health to approach to create lasting wellness in our patients.
This integrated approach reduces our patients’ reliance on medication. The Integrated Care Program teaches patients about the complex bio-psycho-social causes of disease. It also helps them initiate changes in a manageable and meaningful way. MCHC clinicians experience improved patient outcomes through the support of the Program’s team.
We offer a well stocked pharmacy at our Hillside location in Ukiah staffed by our experienced and knowledgeable pharmacist, Gary Wakeman. Operating a pharmacy on-site helps broaden our patients' access to affordable prescriptions and decrease transportation barriers. Whether patients are managing a chronic disease or treating a bout of the flu, our full-time pharmacist is there to help them get the medication they need.
We have developed strong alliances with specialty care providers in all our communities, expanding our patients' access to appropriate care to meet their unique health challenges.
